Verification of benefits (VOB)
A verification of benefits (VOB) is the check an admissions team runs with a prospective patient's insurer to confirm what the plan covers for a level of care and what the patient is likely to owe, before admission.

What it means
A VOB usually confirms that the plan is active, whether the facility is in or out of network, the deductible and out-of-pocket amounts already met, and whether a level of care needs prior authorization. Centers try to run it while the family is still on the phone, or soon after a web form arrives.
A VOB is an estimate, not a promise to pay. HealthCare.gov makes the same point about preauthorization: a decision that a service is medically necessary is not a promise the plan will cover the cost. Coverage for the stay itself is then managed through utilization review.
On many treatment center websites the VOB form is the main conversion point, the moment an anonymous visitor becomes a named inquiry.
Why it matters for your website
Ask only what admissions needs to call back: a name, a phone number and the insurer. Member ID and date of birth can wait for the call. Every extra field costs submissions from people who are frightened and in a hurry.
The data on that form is about paying for addiction treatment. Keep advertising pixels off the page, send submissions only to systems covered by a business associate agreement, and remember that 42 CFR Part 2 counts anyone who has applied for treatment at a covered program as a patient.
